Skin Disorders Associated with Activities for the Prevention of Spreading of Foot-and-mouth Disease

Abstract

The first case of a cow infected by foot-and-mouth disease virus was confirmed on 20th of April in 2010 at Tsuno town in Koyu county, Miyazaki prefecture. The foot-and-mouth disease spread rapidly in Miyazaki prefecture. A total of 292 farms at 5 cities and 6 towns were affected, and about 290,000 domestic animals were sacrificed. In connection with activities associated with the prevention of the epidemic, data on 50 cases of cutaneous or mucosal injuries that had occurred were collected by a questionnaire survey among dermatologists in Miyazaki prefecture. The ages of patients ranged from 20 to 75 years, and the average age was 42.0 years. Forty-five males and 5 females were involved. The numbers of patients and the months in which accidents had occurred were as follows : 17 cases in May, 22 in June, 7 in July and 1 in August. With respect to the kinds of job of patients, 32 patients were public officers living in Miyazaki prefecture, and this number was the highest. The numbers of the patients and their respective disorders were as follows : 46 patients had chemical burns, 2 acute conjunctivitis, 1 miliaria-like eczema, 1 exacerbation of atopic dermatitis, 1 sting injury by needle of syringe, 1 cellulitis, 1 insect bite, 1 caterpillar dermatitis and 1 herpes zoster, induced after the activity for the prevention of epidemics, respectively. In addition, 3 patients had 2 skin diseases and one had 2 skin- and 1 mucosal disorders. As for the places of occurrence of the accidents in 46 patients with chemical burns, 18 patients were injured during the activities of sterilization of cowshed and/or pigsty, 25 at unknown places, 1 in a chicken house and 2 during the work of burying the sacrificed animals, respectively. With respect to the causative chemicals, 23 patients were damaged by slaked lime (calcium hydroxide ; Ca(OH)</sub>2</sub>), 4 by sodium carbonate (Na2CO3), 19 by unknown causes. As for the body regions affected, 5 patients were damaged on their faces, 3 on brachia, 14 on forearms, 6 on hands, 17 on thighs, 4 on knees, 51 on legs and 2 on feet. Some patients had lesions in several areas. In cases with chemical burns, most patients had deep dermal burns. The mechanism of chemical burns in our cases was supposed to be as follows : The chemical agents had penetrated onto the skin surface directly through the nonwaterproof protective coat, or cuff and broken areas of the coat ; in others, the agents had spread over the skin with sweat and infiltrated into the skin more deeply, especially through erosive or ulcerative lesions caused by the friction of rubber boots or from the thighs rubbing against each other.
