A Case of Metastasis of Renal Cell Carcinoma 15 Years after Surgery

Abstract

An 81-year-old man with a history of left nephrectomy for renal cell carcinoma, which was diagnosed in 1992, came to our clinic complaining of a skin nodule on his left forehead 15 years after surgery. The nodule was 7mm in diameter and appeared clinically like a pyogenic granuloma. The patient refused to undergo surgery at that time and chose to be treated by liquid nitrogen cryotherapy, which failed to cure the symptom completely for 4 years. However, in 2011, he started to complain of hemoptysis and multiple metastases were suspected on a CT scan in his right lung, right kidney, right iliac muscle, left major psoas muscle and abdominal wall. In 2012, the nodule on his forehead was resected, which was diagnosed as clear cell carcinoma histologically. To summarize, skin metastasis of renal cell carcinoma can be found several years after surgery. It is necessary to recognize skin metastasis as a crucial differential diagnosis when encountering a patient with a red skin nodule without clear subjective symptoms, which may turn out to be clear cell carcinoma histologically.
