Abstract

A 67-year-old female, originated from Okinawa, developed a nodule on her right nose over the course of 1 month, which became reddish and enlarged after she punctured it herself. On her first visit, she presented a dome-shaped red nodule of 9 mm in diameter on her nasal ala. The tumor was totally resected under the diagnosis of pyogenic granuloma. The histopathological examination revealed the proliferation of spindle-shaped cells and slit-like structures filled with erythrocytes in the dermis. Tumor cells were positive for CD31, D2-40, and LANA-1. HHV-8 was detected by PCR from the operative specimen. Testing for HIV antibody yielded a negative result. The patient had been receiving steroid and cyclosporine for the treatment of diffuse alveolar hemorrhage for 1 year. We therefore diagnosed this case as iatrogenic Kaposi's sarcoma. During the course, a bright red plaque of 2 mm in diameter re-appeared on her left cheek, which showed similar histopathological findings. We asked the attending physician to reduce the dose of immunosuppressive agents, and neither recurrence nor new lesions have been observed. In this case, clinical diagnosis of Kaposi's sarcoma was difficult because the lesion appeared on the face and resembled pyogenic granuloma. We therefore confirmed the usefulness of LANA-1 staining for the diagnosis of Kaposi's sarcoma. Although non-AIDS-related Kaposi's sarcoma is rare in Japan, Okinawa exceptionally appears to be an endemic area of classic type Kaposi's sarcoma. We should bear this rare disease in mind.
