From 1979 through 1998, 13 patients with the posterior pharyngeal wall cancer (PPWC) underwent curative radiation therapy. Eight of them were classified as T1+T2, and 3 patients were lymph node positive. Local recurrence was most frequent followed by neck node recurrence. The 2-year local recurrence free survival rate was 46%. In T1+T2 primaries, at least 68 Gy seems to be required to obtain local control. Three out of 4 nodal recurrences were found in the retropharyngeal lymph node. Synchronous as well as metachronous cancers were seen in a high frequency of the 46%(6/13) and 2 patients succumbed to metachronous esophageal cancer without PPWC recurrence. Two of the 13 patients had a preceding history of neck radiation therapy for tuberculous lymphadenitis. In T1+T2 PPWC, curative radiation is indicated with an adequate dose and wide fields including the base of skull to the lower neck.
