A Case of squamous cell carcinoma from pyodermia chronic glutealis

Abstract

A 56-year-old man presented with a history of pyodermia chronic glutealis since about 20-year ago. Squamous cell carcinoma was diagnosed as a result of a biopsy, since the cauliflower-like tumor was in the center of his right buttock. A whole body examination revealed no evidence of lymphatic and visceral metastasis. We performed sentinel lymph node biopsy of the right groin, and excision of the pyoderma, and wide excision of the squamous cell carcinoma, and stuck artificial dermis onto the defective part of the skin. As a result of the pathology, we found there was no lymph node metastasis ; we diagnosed pT3N0M0, Stage III. We performed simple suture and a mesh skin graft to the defective part of the skin. Since a complete excision of the tumor was carried out during the operation, we did not perform preventive lymph node dissection, chemotherapy, or radiotherapy. During a post-operative follow-up over a one-year period, neither local recurrence nor metastasis were evident. Squamous cell carcinoma which occurrs from pyodermia chronic glutealis is worsened by the prognosis. If pyodermia chronic glutealis passes for a long period of time, we should perform a curative surgical operation at an early stage bearing in mind that squamous cell carcinoma may occur.[Skin Cancer (Japan) 2014 ; 29 : 24-28]
