Abstract

A 44-year-old man was admitted to our hospital with difficulty of swallowing. The gastroendoscopy showed a type 3 advanced gastric cancer in the cardiac region invading to the lower esophagus. Biopsy specimen showed the poorly differentiated adenocarcinoma by microscopy. Abdominal CT revealed bulky regional lymph node metastases.<BR>Neoadjuvant chemotherapy was initiated using S-1+CDDP. Total gastrectomy with D2 lymph node dissection was performed after two courses of S-1+CDDP therapy. Histopathological findings of the resected specimen revealed that tumor cells were formed into solid cystic nests. PAS positive deposits were observed in the tumor nests. According to the histopathological findings, diagnosis was changed into basaloid carcinoma of the esophagus invading to the cardia of the stomach. Twenty four months after surgery, the patient is still in good condition without any recurrence. Here, we report a rare case that S-1/CDDP neoadjuvant chemotherapy was effective for basaloid carcinoma of the esophagus.
