Description
A 71-year-old man was referred to our hospital with the diagnosis of cStage Ⅳ gastric cancer with the findings of U, Less, type2, por+tub2, cT4b (SI), cN3, cH0, cP0, cM1 (LYM). Because overexpression of HER2 protein was observed in the immunohistochemical staining of the primary tumor, combination chemotherapy of Capecitabine + Cisplatin + trastuzumab was performed as a first-line chemotherapy. After six cycles, CT scans revealed that the size of the primary tumor and metastatic lymph nodes were considerably decreased, suggesting a partial response (PR). We judged that the curative resection was possible and performed total gastrectomy with D2-No.10 lymph node dissection. Postoperative histopathological examination revealed that the gastric cancer had therapeutic effect of Grade 2 with the findings of Type5, tub2, ypT2 (MP), ly0, v0, ypN0, ypStage IB. We presented a case of HER2 positive gastric cancer with esophageal invasion, successfully treated by trastuzumab with chemotherapy followed by surgery.
