Determination of causative bacteria is essential for management of pneumonia in children. If the offending pathogens are accurately defined, an early recognition and prompt institution of appropriate therapy may offer the best chance to reduce severity. Thus, several culture methods have been utilized to recover the bacterial pathogen of pneumonia.Among these methods, washed sputum culture is one of the most useful techniques of detection of real pathogenic bacteria. Gram stain smear microscopy of washed sputum is also valuable for evaluation of quality of sputum and for rapid estimation of pathogens, aiming at guiding quickly initiated antibiotic choice. In addition, this method is able to differentiate between bacterial associated pneumonia and non-bacterial associated pneumonia.Sputum can be obtained non-invasively from pediatric patients. Therefore, washed sputum culture and gram stain should be encouraged in clinical practice for management of pneumonia in children.
