A case report: gabapentin improved medication-overuse headache

Medication-overuse headache (MOH) is an intractable disease that results from taking headache treatment drugs frequently. We report a case of MOH induced by multiple medications and alleviated by gabapentin. The patient was a 50-year-old woman who had chronic tightened headache. Her migraine began to occur in her junior high school days. Twelve years ago, she began to take compound analgesics daily. Six years ago, she began to take rizatriptan daily and loxoprofen more than 10 days a month, and she then visited our clinic. We fi rst educated her about MOH and instructed her to discontinue rizatriptan and loxoprofen. Five weeks later, she could withdraw from these drugs, and we started to administer gabapentin. Eleven weeks later, her headaches began to shift from tightened to pulsatory, and they then improved. It seems quite likely that gabapentin inhibited brain excitability induced by an overuse of headache treatment drugs, resulting in the improvement of MOHs. Patient education for MOH was also useful for withdrawal from overuse of medication.
