Experience of the Miyagi Trial helped to shape the planned national mammography screening program which started in 2000. Surgeons, gynecologists and general practitioners other than radiologists, have long been involved in the clinical breast examination program in Japan. Therefore, it was essential to design an education program before starting mammography screening. We asked members of the Sendai Medical Association to take a two-day, 16-hour course, followed by quality assessment examination ; the interpreting physicians were required to have adequate levels of sensitivity and specificity for mammogram categorization at the second mammography check.<BR>The other issues related to maintenance of a quality standard were mammography equipment and a training program for photographers. The mammography machines needed to satisfy the guidelines of the Japan Radiological Society, and also had to yield adequate images which met stringent quality levels, an aspect that included the role of radiographers. Further efforts should be made at every step in mammography screening by general practitioners in collaboration with radiologists and radiological technologists to improve quality control.
