Previous studies using data on health insurance claims in Japan could not resolve the problem, "How long after implementing the health policy to raise the coinsurance rate should we evaluate it?" The fact infers that no studies declare the effect of the increase in the coinsurance rate on the rate of doctor's consultation. We focused on the change in the cost-sharing rule of Employee's Health Insurance in September 1997 to make an evaluation on health insurance policy. We employed econometric time series techniques to develop recursive vector autoregressive models in order to analyze the effect of unobserved shocks on the outpatient. The behavior of the patient in response to the shocks can be represented by plotting the impulse response functions.<br>The results derived from empirical analysis over the sample period from 1993:01 to 2003:03 are as follows. First, the increase in the effective coinsurance rate significantly reduced the rate of doctor's consultation. Second, a price shock to an outpatient accounts for about 50 percent of the forecast error variance of health care expenditure of outpatients per case from the three months to five months after. The contribution ratio of the shock can be useful for the forecast in the short run. Third, the response of health care expenditure of outpatients per case to the change in the rate of doctor's consultation reduced during 10 months, but the response of the rate of doctor's consultation to a price shock did not converge in a year.<br>It is concluded that we can evaluate the health policy that raises the coinsurance rate after the first year. When we examine the effect of the change in the health care system by using individual data, we must pay attention to the sample period with great prudence.
