Abstract

<p>This case report describes the treatment of a male patient who has autism aged 11 years and 9 months old having prolonged retention of maxillary second primary molars, ectopic eruption of maxillary second premolars and maxillary protrusion. The developmental quotient of the patient was 59 by the Enjoji infant analytic developmental test, and cooperation with dental treatment was not low. The patient received periodical oral health care in dentistry of the Medical and Welfare Center for adaptation improvement to dental treatment from 5 years and 11 months old.</p><p>The initial treatment plan was to move the maxillary second premolars to correct the positions for dental health. If the occlusal guidance was completed safely and the patient wished to receive orthodontic treatment, we planned to refer the patient to an orthodontic clinic for treatment of maxillary protrusion.</p><p>A dental splint was made and the patient was instructed to wear it at home to confirm whether the patient could put on the appliance in the oral cavity. After three months, a fixed appliance (lingual arch) was made and placed in the oral cavity because the duration of wearing the dental splint was long. Seven months later, the maxillary second premolars were moved in dentition and the appliance was removed because oral health care had become easy. Occlusal guidance was successful, and the patient was referred to the orthodontic clinic because the patient and parents wished to improve the maxillary protrusion. The chronological and mental ages of the patient at the time of the orthodontic clinic consultation were 13 years old and 7 years and 8 months old, respectively.</p><p>This case report suggests that autistic children who are able to undergo adaptation improvement to dental treatment can start orthodontic treatment.</p>
