- Chemiluminescent magnetic particle immunoassay (CLIA) 法を用いたペプシノゲン法
- 職域及び人間ドックにおける胃健診
Description
血中ペプシノゲン (PG) 低値群を胃癌高危険群とするPG法を行った。血清PGは免疫発光測定装置を用いたChemiluminescent magnetic particle immunoassay (CLIA) 法で測定し, Radioimmunoassay (RIA) 法による測定値との相関及び判定の一致率をみた。対象は1502名 (男/女=872/630, 平均年齢40.8歳) で, PGI≦70ng/mlかつPGI/PGII比 (I/II比) ≦3をPG法陽性とした。CLIA法とRIA法でPG値の相関係数 (r) はPGIで0.989 (p<0.0001, n=509), PGIIで0.994, I/II比で0.980といずれも強い相関を示し, 判定も97.6%が一致した。PG法により早期胃癌1名 (51歳, 男性) が発見され, 胃癌発見率は0.08%であった。血清PG値の測定法として, CLIA法は, 従来のRIA法によるものと測定値が極めてよく相関し, RIA法で積み重ねられた経験と結果をそのまま応用が可能と考えられた。
