Perfusion imaging by MDCT (perfusion CT) is a new modality to measure tissue perfusion of the pancreas. We investigated pancreatic blood flow by perfusion CT in patients with acute pancreatitis to clarify whether perfusion CT can detect pancreatic necrosis in the early phase of acute necrotizing pancreatitis (ANP). Perfusion, peak enhancement intensity (PEI) and blood volume (BV) of the pancreas in acute pancreatitis were significantly low compared with those in healthy controls. Perfusion, PEI and BV were significantly lower in poorly perfused areas of the pancreas in ANP compared with those in acute edematous pancreatitis (AEP) on admission. 2 weeks later, the perfusion in AEP had recovered to almost normal, whereas perfusion in ANP decreased and pancreatic necrosis was observed in accordance with the poorly perfused area on admission. Perfusion CT is useful for detecting pancreatic necrosis in the early phase of the disease.<br>
