This study was designed to investigate of serial changes in facial expression following cerebrovascular disease (CVD). Patients with CVD were divided into two groups : 19 with occlusive CVD and 11 with hemorrhagic CVD. To evaluate the facial expression, facial photography was undertaken serially from the onset to discharge. This examination of facial expression included 10 categories, by which the mental and psychological state could be judged. Each item was graded from 2 (positive) to 0 (absent). A score for the facial expression was obtained by averaging all of the scores for the 10 items at each time of facial photography. The facial expression revealed higher scores during the acute stage both in occlusive CVD and in hemorrhagic CVD, followed by a gradual decrease during the chronic stage. The facial expression scores in hemorrhagic CVD tended to be sustained at a higher level compared to those in occlusive CVD. During the acute stage of occlusive CVD, the facial expression scores obtained from patients with a Japan Coma Scale (JCS) of 03 were significantly lower than those of patients with JCS 1030, whereas hemorrhagic CVD patients both with JCS 03 and with JCS 1030 demonstrated higher scores. During the chronic stage, patients achieving a good recovery revealed significantly lower scores for their facial expression than did those with a poor recovery. Serial changes in facial expression following CVD may be due in part to the pathogenesis of CVD.
