A case report: The effect of Darubepoietin in a 64-year-old man on hemodialysis with pure red cell aplasia and adrenal dysfunction

Abstract

A 64-year-old male on long-term hemodialysis due to IgA nephropathy underwent bilateral nephrectomy in 2000 and 2005 because of renal cell carcinoma. Post operatively, he showed progressive severe anemia and hypotension. To diagnose and treat severe anemia, he was admitted to our hospital in December 2007. Laboratory blood test showed that Hb and Ht decreased to 5.2g/dL and Ht 15.9%, respectively. On bone marrow biopsy, the patient was diagnosed as having pure red cell aplasia. The patient required several blood transfusions because his anemia was resistant to erythropoietin. Therefore, we changed from Epoetin β (9,000U/week) to Darubepoietin (120mg/week). As a result, anemia was improved, and Hb and Hct levels became 8.1g/dL, and 24.4%, respectively. In this report, a patient with a long history of hemodialysis who developed erythropoietin-resistant anemia and adrenal dysfunction after renal surgery was successfully treated with Darbopoetin.
