- Inverted type TCC in the renal pelvic with inverted papilloma-like features: a case report

Background: Inverted TCCs, which are unfamiliar to many cytopathologists, should be differentiated from IPs. Although IPs with malignant transformation or with coexisting TCC have been described, some regard these as inverted TCCs. A case of inverted TCC with IP-like features arising in the renal pelvis is reported.<BR>Case: A 75-year-old man noticed hematuria. CT and MRI revealed a smooth-surfaced tumor in the right renal pelvis. Cytology of voided urine showed atypical cells with irregularly shaped-nuclei and increased chromatin scattered or formed clusters. With a diagnosis of grade 2 TCC, a right nephrectomy was done. Macroscopically, the tumor was a polypoid mass with a smooth surface. Pathological examination showed that 1) the tumor surface was covered with a few layers of urothelial cells without atypia, 2) tumor cells without atypia showed an inverted growth pattern, 3) exophytic papillary proliferation of atypical cells was consistent with those of ordinary grade 2 TCC, 4) transition from the IP-like region to the ordinary TCC was observed, and 5) in some foci, atypical cells showed an inverted growth pattern. We diagnosed the current case as inverted TCC with IP-like features.<BR>Conclusion: Inverted TCC should be included in the differential diagnosis of urothelial tumors showing inverted growth patterns.
