Abstract

Hypopharyngeal cancer is mostly diagnosed at the advanced stage and resected as total pharyngo-laryngo-esophagectomy in many cases. However, as long as the tumor is localized, there are some cases of partial resection of larynx and hypopharynx. In these cases, the following is considered to improve the quality of life of the patients;<br>1. Relief of physical burden on surgery<br>2. Early oral intake of meal<br>3. Early discharge and social return<br>4. Conservation of phonatory function<br>5. Complete conservation of laryngeal function<br>As for hypopharyngeal cancer localized in unilateral piriform sinus or posterior cricoid, it is possible to reconstruct hypopharynx and cervical esophagus all at once by suturing hypopharyngeal mucous membrane with myo-mucosal flap of remaining larynx of which the cartilages have been extracted. This procedure enables patients to be relieved of physical burden and to early take meals orally, so, early discharge and social return can be expected.<br>In terms of complete conservation of laryngeal function, postoperative aspiration holds the key.<br>In case of hypopharyngeal cancer localized in unilateral piriform sinus, it is possible to conserve laryngeal function completely by means of laryngectomy using some flaps.
