Twenty-seven cases of gingival cancer of the mandible were treated with a surgical procedure based on evaluation of the depth of invasion into the buiccinator or sublingual spaces. In the buccal site, the depth of invasion was evaluated by ultrasonography and was classified into three categories.<br>Class I: submucosal invasion<br>Class II: invading to the buccinator muscle without destruction of the fascia of the cutaneous site<br>Class III: invading to the buccinator muscle with associated destruction of the fascia of the cutaneous site<br>The surgical margins were delineated as follows.<br>Class I: the margins were delineated in the buccinator space<br>Class II: the buccinater muscle was resected, including the fascia of the cutaneous site<br>Class III: the tumor was resected, including the skin of the cheek<br>In the lingual site, when the tumor invaded to the mucosa of the mouth floor, dissection of the sublingual space was performed. No local recurrence was found in any case.
