We have reported that 3-cm or larger lymph node metastasis (CN3) is one of the significant risk factors for patients with papillary thyroid carcinoma (PTC) when the patient is 50 years or older. In the present study, we retrospectively analyzed the prognostic significance of the size and number of lymph node metastases for 604 patients with PTC. By multivariate analysis for disease-specific survival, CN3 was the significant factor (risk ratio=5.3) in 50-years-old or older patients. Among 60 patients with CN3, 20 (33%) died of the disease (12 died from distant metastasis and 4 died of repeated nodal recurrences). On the other hand, 5 or more lymph nodes with pathologically proven metastasis (PN5) were significant (risk ratio=3.5) for disease-free survival of patients younger than 50. Of 115 patients with PN5 (age<50), 18 (16%) had recurrence in the regional lymph node. From these results, we conclude that CN3 is related to cancer-specific death in old patients due to distant metastasis and repeated nodal recurrences, and that PN5 is associated with high nodal recurrence rate in young patients.
