Impact of APACHE Ⅱ and SOFA Scores on the Clinical Outcome of Patients with Hepatic Portal Venous Gas

Objective: The aim of study was to retrospectively review the clinical features of patients with hepatic portal venous gas (HPVG) and evaluate the significance of The Acute Physiology and The Chronic Health Evaluation II (APACHE II) score and the Sequential Organ Failure Assessment (SOFA) score in the outcome of HPVG. Methods: Between 2005 and 2012, 19 cases of HPVG were treated in our hospital. We evaluated the relationship between mortality and clinical data including the APACHE II and SOFA scores. Results: Seven cases underwent surgery, and twelve cases received conservative treatment. The cause of HPVG was intestinal ischemia in nine cases including seven cases of non-occlusive mesenteric ischemia, enteritis in three cases, adhesive ileus in two cases, iatrogenic in one case, and idiopathic in three cases. The overall mortality was 21.1%. The APACHE score, SOFA score, the value of lactate and the presence or absence of shock were significant factors when the outcome was compared. Conclusion:The APACHE Ⅱ score and SOFA score might be useful to predict the prognosis of patients with HPVG and also to determine the need for surgery.
