Description

Case. An 80-year-old man presented with productive cough. Chest CT scan showed a tree branchlike shadow (bronchial cast shadow) in the apical posterior segment of the left upper lobe. The entire bronchial cast shadow showed hypermetabolism on ^<18>F-fluorodeoxyglucose positron emission tomography(FDG-PET) images. Bronchoscopic examination revealed a smooth polypoid tumor obliterating the left B^<1+2>, which was diagnosed histologically as carcinoid. A left upper lobectomy was performed and pathological examination revealed that the tumor grew in a polypoid manner in the left B^<1+2>, proliferating intraluminally in a peripheral direction along the bronchial branches. In parts, it invaded the pulmonary parenchyma beyond the bronchial wall. Conclusion. It is interesting to note that the entire bronchial cast shadow showed hypermetabolism on FDG-PET images. We reported a comparatively rare case of bronchial carcinoid tumor showing bronchial cast shadow due to endobronchial growth.
