Background. Anastomotic dehiscence after bronchoplasty can be fatal. Case. A 69-year-old man was admitted to our hospital. Right sleeve upper lobectomy was performed due to invasive metastatic #11s lymph node. A pedicled pericardial fat pad flap was wrapped around the anastomotic site, and the postoperative course was uneventful. When the patient was readmitted for adjuvant chemotherapy, he had a high fever. Laboratory data showed elevated CRP. Bronchoscopy revealed a partial anastomotic dehiscence. Chest CT showed a pericardial fat pad between the pulmonary artery and a free space caused by partial anastomotic dehiscence. Neither pneumothorax nor empyema occurred. The patient recovered after intravenous antibiotic administration. Chest CT 10 months later showed no space around the anastomosis and no bronchial stenosis. Conclusion. We report a case of partial anastomotic dehiscence after bronchoplasty that improved without progressing to a critical condition, probably owing to the pedicled pericardial fat pad flap wrapped around the anastomotic site at operation.
