Successful Bronchoscopic Closure of Bronchial Stump Fistula Using Cyanoacrylate

Background. Several treatment methods for postoperative bronchial stump fistula, including bronchoscopic therapy, have been proposed, but no standard treatment method has been established so far. Case. A 79-year-old man underwent right middle and lower lobectomy for squamous cell carcinoma. He received postoperative irradiation for microscopic residual carcinoma cells at the bronchial stump. Bronchoscopic examination performed 1 year after surgery revealed a pin-hole sized fistula in the bronchial stump. Chest drainage was not successful; therefore, we performed an open thoracic window. At 1.5 years after the second surgery, a bronchoscopic closure using cyanoacrylate was successfully accomplished. Three months after bronchoscopic treatment, no recurrence of the bronchial fistula was observed. Furthermore, we plan to perform muscle plombage after sterilization of the pleural cavity is achieved. Conclusion. Our study suggests that bronchoscopic closure using cyanoacrylate is an effective technique for the closure of small-sized fistulas in the bronchial stump.
