Description

Anticoagulation management during cardiac surgery is difficult in patients with heparin resistance. Here we report a patient with heparin resistance who underwent on-pump cardiac surgery with the use of argatroban during cardiopulmonary bypass (CPB). A 44-year-old man with a history of hypertension and hyperlipidemia was scheduled for aortic valve replacement. His platelet count was 108×104. The activated clotting time (ACT) remained below 400 seconds despite 500 IU/kg heparin and antithrombin supplementation. Argatroban was selected for anticoagulation during CPB. Argatroban was started with a bolus dose of 0.25 mg/kg, followed by an infusion of 5-8 μg/kg/min. ACT was maintained over 400 seconds during CPB. No complications such as thrombosis or excessive bleeding were observed either during CPB or the perioperative period.
