Mechanical patient-controlled analgesia (PCA) is an effective method of pain relief after surgery. There are advantages and disadvantages to the use of mechanical PCA. The advantages of mechanical PCA over normal PCA are that fine adjustments of parameters are possible and a record is automatically saved. The disadvantage is that it requires more effort than normal PCA. For example, mechanical PCA requires 1) preparation of the equipment and drugs, 2) observation of the patient's condition, 3) changing various settings, and 4) data extraction. Therefore, the perioperative care team is important because several factors are involved. In our hospital, five mechanical PCA pumps have been in operation since 2009, to provide pain relief after inflammatory bowel disease surgery. In this report, we describe the following : 1) the characteristics of the mechanical PCA pump ; 2) the process we have initiated to ensure the use of mechanical PCA in our hospital ; 3) the role of the clinical engineer in appropriate management of PCA ; and 4) an example of a problem that arose during PCA use.
