説明

A 29-year-old woman who had s history of livedo reticularis on extremities for two years presented pruritic painful ulcers on ankle joints and dorsal aspects of feet in summer. Clinical feature of the eruption appeared as atrophie blanche, in which central ulcer showed ivory scar with peripheral telangiectasia, erythema, and pigmentation.
Laboratory examination including hematological and biochemical analysis were within normal limits. Histopathological study of the surrounding erythematous region of the ulcer was done.In the upper dermis, there were many dilated blood vessels and the walls of them showed fibrinoid degeneration. In middle to deep dermis, blood vessel walls were thick and occuluded with fibrin deposits. Inflammatory cell infiltration around peripheral blood vessels was not so remarkable.
From clinical and histopathological findings, we diagnosed livedo reticularis with summer ulceration.
Treatments with topical corticosteroid and oral anti-inflammatory-pyretic drug were very effective, resulting into clearance of the lesion within about a month.
