Abstract

The natural course of hepatitis C virus (HCV) infection has not been systematically studied. To investigate the frequency and factors of spontaneous elimination of HCV in chronic carriers, we conducted a retrospective study on 278 chronic HCV carriers without antiviral therapy between April 2006 and February 2014. The mean follow up duration was 5.8 years. Serum HCV RNA was spontaneously eliminated in 4/278 (1.44%). Thus, the incidence of spontaneous negativation of HCV RNA was 0.24%/year/person. Univariate analysis revealed that low values of serum HCV RNA, ALT, γGTP, total bilirubin, AST to platelet ratio index (APRI), and a high value of platelet count were associated with spontaneous viral elimination. IL28B single nucleotide polymorphisms (rs8099917, rs11881222, rs8103142) of all the four patients who cleared virus were major alleles. In addition, every spontaneous HCV clearance was observed after surgery or drug induced hepatitis or colonic diverticular hemorrhage.
