Abstract

We report a case of the resection of a liver tumor thought to have arisen from metastasis of a pancreatic neuroendocrine tumor (NET). The 58-year-old male patient had undergone pancreatoduodenectomy for a tumor of the pancreatic head 19 years previously in our hospital. The tumor was diagnosed as pancreatic islet cell cancer. The patient recovered without recurrence and was monitored afterwards by local doctors. One year ago, an abdominal CT identified a liver tumor 10-mm in diameter and the patient was referred to our hospital because the tumor appeared to be developing. A CT scan revealed a mass 20-mm in diameter in S6 of the liver with the effect of the contrast medium lasting through to the delayed phase.<br> Hand-assisted laparoscopic partial resection of the liver S6 was performed and histopathological examination revealed that the tumor was composed of cells with round nuclei arranged in sheets or rosettes. The tumor cells were stained positive using immunohistochemical staining protocols chromogranin A and synaptophysin and the tumor was diagnosed as a NET. Differentiation from a primary hepatic NET was difficult, but it was considered that this case was a liver metastasis of the original pancreatic NET. This appears to be a rare case of recurrence more than ten years after the primary NET resection.<br>
