Abstract

The low caloric intake test was performed in a case with Dubin-Jonson syndrome who did not demonstrate the secondary rise in the BSP plasma clearance test. Following the fasting test the total plasma bilirubin increased by almost 100% (mainly direct type), and a a remarkable improvement of ICG clearance was noted. Interestingly, there occurred the secondary rise in BSP in plasma after fasting.<BR>The physiological alteration which causes the secondary BSP rise fbllowing the fasting seems to be dependent on the basic metabolic defect (characteristic in Dubin-Johnson syndrome) involving excretion of conjugated BSP into bile canaliculi.<BR>Furthermore, the demonstrated concomitant improvement in ICG uptake by hepatocytes may provide additional support to the concept that uptake and excretion of anionic dyes in hepatocyte are relatively independent steps on which certain selectivity is superimposed.
