A 55 year old male was found to have anemia and splenomegaly and esophageal varix. Liver function tests were normal and the serum HBs antigen was positive. Spleno-caval shunt with splenectomy was performed. Histology of the liver revealed chronic hepatitis. The spleen was 720 gm in weight. Soonly thereafter, Eck's syndrome appeared and marked elevation of serum transaminase and jaundice followed. He died of hepatic coma five months after the surgery. At autopsy, the liver was atrophic (900 gm) and histology showed the subacute hepatic necrosis, characterized by presence of regenerated nodules of liver cells, broad band-like proliferation of connective tissue in which marked proliferation of bile ductules and infiltration of lymphocytes were seen. Also found were orcein stain positive inclusion bodies in the liver cell. Above findings were consistent with massive necrosis, presumably due to an activation of viral hepatitis, leading to the final stage. The present case illustrated that hepatitis in Banti's disease can be fatal when triggered by some unknown factor.<BR>Banti's disease, HBs antigen, chronic hepatitis, subacute hepatic necrosis
