Bucolome (900 to 1800mg/day) was administered orally for 1 to 4 weeks to 14 patients (7 males and 7 females) with the Dubin Johnson syndrome in the cluster area of the syndrome in Japan. The drug failed to reduce the hyperbilirubinemia in these patients. On the contrary, Kumagai and others reported that bucolome was very effective in decreasing the plasma bilirubin level in one patient with the Rotor type hyperbilirubinemia.<BR>We could not identify even one case of the Rotor type hyperbilirubinemia among 168 sibs consisting of 26 sibships in 23 families with D-J trait.<BR>The difference in the modes of metabolism of dyes such as BSP and 131-I BSP and the discordance in the histologic findings (photomicroscopic and electron microscopic) in these two constitutional jaundice may suggest that these two disorders are based on the different patho-physiological mechanisms.<BR>All these facts are consistent with the hypothesis that the Rotor type hyperbilirubinemia is not a variant of the D-J syndrome and they belong to different clinical entities.
