A Case of the Primary Hepatoma with Biliobronchial Fistula Diagnosed by Percutaneous Transhepatic Cholangiography

Abstract

The case reported was 65 year-old female, complained with right hypochondralgia and right back pain, lacking of cough and expectoration. Chest X-ray film showed infiltrative shadows in right lower lung. P.T.C. revealed irregular shadows in the liver, which communicated with bronchial tree, through the diaphragm.<BR>Hepatic arterial canulation was done, and infusion therapy of chemotherapeutic agents was followed. Unfortunately she died of cachexia on 71 post-operative days.<BR>In autopsy, cholangiocellular carcinoma occupied in upper lesion of the right hepatic lobe, and severe fibrous adhesion between liver, diaphragm and lung was observed. Microscopically, degenaration of cancer cells and atrophy of muscular layer of the diaphragm were found in some part of lesion.<BR>The present report discussed on the clinical features, pathological findings, therapeutic evaluation of chemotherapy and on ethiological aspect of the biliobronchial fistula formation.
