Ischemic colitis induced on the injection day of pegylated-interferon for persistently normal ALT

Abstract

Various adverse effects by pegylated-interferon/Ribavirin (Peg-IFN+RBV) therapy have been reported. A 47 year-old female without a history of hypertension or diabetes mellitus had left lower abdominal pain on the first day of Peg-IFN+RBV therapy for persistently normal ALT (PNALT) with spontaneous relief. On the second injection day of Peg-IFN, she experienced stronger pain in the same lesion with hematochezia. Colonoscopy revealed 3 lines of longitudinal ulcerations in her descending colon, compatible with ischemic colitis. Her symptom was disappeared after cessation of Peg-IFN+RBV. This case could be interesting because close causal relationship between Peg-IFN and ischemic colitis was observed although there are some reports of ischemic colitis several months after IFN injection.<br>
