We describe a case of elephantiasis nostras with akatsuki disease in a 49-year-old man. At the age of three, he suffered open fractures of his left foot and ankle due to a traffic accident. Even after treatment, the wounds were often inflamed and occasionally discharged pus. On his first visit to our hospital, he had not washed his leg at all for the preceding 10 years. His entire left foot was swollen with solid verrucous nodules and tumors that were covered with green crusts measured from 0.5 to 2 cm in size. Histopathological examination revealed that thick collagen bundles were present in the dermis, in theupper part of which, dilated lymph vessels were abundant. A diagnosis of lymphedema was made, and clinically, the lesions were diagnosed as elephantiasis nostras with akatsuki disease as a result of the extended period that he did not wash his leg.
