We report here two cases of anaphylaxis, of which we suspected tropomyosin-related protein as a causal antigen. Case 1; A 26-year-old man developed anaphylaxis after taking chikuwa which is usually made of mashed cod. He had also experienced dyspnea after taking shrimp or crab since childhood. Case 2; A 60-year-old woman developed generalized flush and hypotension with anaphylaxis after taking octopus and squid. Radioallergosorbent test for Ig E revealed that both patients had specific Ig E for shrimp, crab, squid, cockroach, roundworm, anisakis, and house-dust mite. By skin test, both patients strongly responded for shrimp, crab, squid, and anisakis. Immunoblot analysis revealed that both of patient sera reacted with 34kd anisakis antigen. Based on these findings and literatures, we speculate that there is a possible cross-reactivity among antigens in these creatures which are known to have an identical allergen, tropomyosin.
