Abstract

A case of hydroa vacciniforme that started at the age of 2 years with erythema and papule and healed with scar formation was followed up from the age of 13 to 21 years. When he was first seen by us, he showed only crusted eroded areas and marked scars on the nose, lips, ears, backs of hands and fingers and foreams and porphyria rather than hydroa was suspected. During the observation, however, discrete erythematous papule with central umbilicus appeared on several occasions a year, especially when he was exposed to the sun for a long time. The episode was noted more often in spring, summer and autumn but also noted in winter when he was irradiated by reflected light from snow. Minimum erythema dose (MED) to ultraviolet (UV) A and B was normal but MED to monochromatic light was slightly lowered in the range between 300 and 370nm. Erythema produced by 3 MED of UVB showed early histological change of hydroa vacciniforme which was composed of intracellular edema, dyskeratotic cells and inflammation in the dermis. Laboratory examination showed low titer of positive speckled type antinuclear antibody. Porphyrins in the urine, feces and blood were normal. Skin symptom has not been seen since the age of 16 years.
