抄録

Successful treatment of a patient with extensive aganglionosis is reported regarding the operative procedure and its outcome. We initially attempted a modified Kimura's colostomy in which the aganglionic terminal ileum was anastmosed longitudinally with the ganglionic ileum in an attempt to save the alimentally enterohepatic circulation. Secondary, Maritn's operation was attempted with patch anastmosis of the descending colon to assist in the intraluminal reabsorption of water. During the early postoperative course, an oligopeptid diet was necessary for the patient to gain weight. However, the patient is now in almost normal habilitation at his present age of 8 years. We would emphasise that all of the aganglionic segment in the patient with extensive aganglionosis can be used to assist in reabsorption if they can be anastmosed in the patch style reported by Shandling.
