Impalement injury is a rare injury often caused by accidental falls or sitting down on pointed objects and the extent of deep organ injury is difficult to access in this trauma. Herein we report a case of anorectal impalement injury caused by a water pistol. A 6-year-old girl accidentally jumped down into a small pool and the internal part of water pistol penetrated her anus. She visited a regional hospital the next day with abdominal pain and fever. She was diagnosed with a rectal perforation by abdominal CT scan and transferred to our hospital. Due to her history, symptoms, and abdominal CT scan, she was diagnosed with a rectal impalement injury. Emergency laparotomy revealed rectal perforation, and she underwent closure of the rectal perforation, peritoneal lavage, and sigmoid loop colostomy. Postoperative barium enema showed another small leakage in the rectum distal to the perforated site. Her colostomy was closed 5 months after the operation. Examination of the lower rectum is important for the treatment of anorectal impalement injury.
