Laparoscope-Assisted Resection of an Abscess of the Round Ligament of the Liver in an Infant : A Case Report

Abstract

A 1-year-old boy with omphalitis was diagnosed as having abscess in the round ligament of the liver on abdominal ultrasonography and CT. On further investigation after improvement of the omphalitis, the ligament measured a maximum diameter of 7.5 mm with a mass formed at a site attached to the liver. No blood flow was observed in the ligament. No causal lesion in the abscess was observed in the liver, biliary tract, or pancreas. The abscess was suspected to recur in the future because the ligament was still swollen even after improvement of the omphalitis. Therefore, we decided to resect the inflamed ligament. Firstly, the ligament was resected via an umbilical incision. Then, we tried detaching the remaining portion of the ligament using a laparoscope. Since bleeding from the ligament occurred near the liver, we extended the incision to obtain a broader operational field for hemostasis. Pathological examination of the specimen confirmed the above diagnosis. Surgical resection of the abscess is required for the radical treatment of this extremely rare disease. The procedure we performed after improvement of the omphalitis might be feasible with careful management of bleeding from the ligament.
