<p>A 3-year-old girl was brought to our hospital with the chief complaint of marked abdominal distension. Imaging studies including CT and MRI showed an intraabdominal huge multicystic mass measuring 20 × 10 × 22 cm. Suspecting an omental cyst or a mesenteric cyst, we performed single-port surgery. A wound retractor was inserted through the umbilical z incision, a sump tube was inserted through a small incision of the cyst wall, and the fluid content was aspirated. Using 3 ports via the umbilicus, a cyst was excised from the omentum and extracted from the abdomen. The histopathological findings confirmed our suspicion of an omental cyst. She showed no recurrence a half year after the surgery. An omental cyst is a rare lymphatic malformation in children, and surgical excision is the principal treatment to prevent acute abdomen. Single-port laparoscopic surgery is less invasive and causes no scarring, and should be recommended as the first line of therapy.</p>
