抄録

The strenuous two-step test which charges 3 times the exercise load of single two-step test to a patient as much as possible in 3 minutes, was devised with the use of a 3-channel radiocardiograph, in an attempt to improve the rate of diagnosis of coronay insufficiency.<BR>Maximal heart rate during the strenuous two-step test ranged about 84 to 87% of maximal heart rate which was reported by Robinson.<BR>Positive responses were seen in 35.1% of 148 cases with cardivascular disease and in 50% of 42 cases with ischemic heart disease, in which the double two-step test was negative. In healthy normal subjects, only 7.1% of 197 cases gave findings interpreted as positive.<BR>In 49.4% of 79 cases with positive responses, the time of appearance of ST depression which was interpreted as positive reaction took place from during exercise to less than 30 seconds. Without the use of telemeter, the positive incidence may become lower.<BR>Complaints of patients during and after exercise loading consisted mostly of mild palpitations and byspnea dizziness in a few cases.<BR>It is concluded that this test is quite useful in the diagnosis of coronary insufficiency undetectable by the double two-step test.
