Phonocardiographic studies were performed to study the incidence of respiratory splitting of the second heart sound in 150 cases of healthy children, aged one month to 14 years.<BR>Furthermore, fifty patients with atrial septal defect (ASD), ventricular septal defect (VSD), rheumatic carditis and acute glomerulonephritis were examined.<BR>Results were as follows :<BR>1. The incidence of physiological splitting of the second heart sound was found to be high as 80 % (120 cases) of healthy children during the period of inspiration, ranging 0.015 to 0.055 sec. of interval time with a mean of 0.031±0.007 (Standard Deviation) sec.<BR>The incidence of splitting phenomena and variation of mean interval were increased as age increased<BR>2. The splitting disappeared in 54 % of above 120 cases during the expiration, and the incidence of disappearance was decreased as age increased.<BR>The interval time of splitting during the expiration varied from 0 to 0.035 sec. with the mean of 0.008 sec.<BR>3. The difference of interval time of splitting on inspiration and expiration showed from 0.015 to 0.03 sec. in majority cases of healthy subjects with a mean of 0.024 sec.<BR>4. Seventy percent of patients with ASD showed fixed splitting.<BR>5. Respiratory splitting was observed in 53 % of cases with VSD. However, no fixed splitting was observed in these cases.<BR>6. Pathological splitting including abnormal splitting by the respiration (should have splitting interval of over 0.035 sec. during expiration) and fixed splitting was observed in 40 % of rheumatic carditis on the initial examination which decreased to 20 % after 2 months of treatment.<BR>7. No difference was observed in the results obtained from the subjects of acute glomerulonephritis as compared with those of healthy children.
