Abstract

This paper deals with two cases of mesenteric panniculitis causing intestinal obstruction soon after laparotomy.
Patient 1, a 52-year-old man who underwent a high anterior resection for cancer of the sigmoid colon on December 25, 2003, developed induration of the surgical wound on postoperative day 7 and had vomiting on postoperative day 11. Following an abdominal CT scan conducted immediately, mesenteric panniculitis causing intestinal obstruction was diagnosed. Administration of dexamethasone was started and it brought a cure on the 28th day after the initiation.
Patient 2, a 54-year-old man who underwent a high anterior resection for rectal carcinoma on August 11, 2004, had vomiting and induration of the surgical wound like in the patient 1 on the postoper-ative day 8. Following an abdominal CT scan conducted immediately, mesenteric panniculitis causing intestinal obstruction was diagnosed. Administration of dexamethasone was started and it brought a cure on the 24th day after the initiation.
Mesenteric panniculitis is a nonspecific inflammatory disease of unknown origin. The disease is thought to be relatively rare, but it has increasingly been reported in recent years. Of these reported cases, cases of early development of the disease after laparotomy are few, only 18 cases in Japan. In this paper we report the clinical course of our case together with some discussion of these domestic cases to elucidate its clinical findings.
