We report a case of hernia in the linea alba caused by a trauma in an aged patient.<br> An 87-year-old man was seen at the hospital because of an enlarging mass at the upper abodominal wall on the middle line. He complained of the sense of discomfort and tenderness around the mass. He had a past history of multiple fractures at the rib in an accident in 1997, and noticed the mass of the abdomen for the first time in 1998. Ultrasound examination and CT scan of the abdomen revealed linea alba hernia. A radical operation was therefore performed. There was a defect about 15×10mm in diameter in the linea alba, and preperitoneal fatty tissue and the hernia sac prolapsed and preperitoneal fatty tissue was incarcerated. As the defect was relatively small, simple closure was performed.<br> Our case was the 15th report of linea alba hernia in the elderly more than 70 years of age in Japan. Of those 15 cases, incarceration and/or strangulation were seen in seven (47%) cases. Accordingly we must consider possible occurrence of incarceration or strangulation when linea alba hernia affects elderly patients.
