Gastrointestinal mesenchymal tumors (GIMT) of the gastrointestinal tract can broadly be classified into myogenic, neurogenic tumors, and gastrointestinal stromal tumors (GIST) which do not differentiate into either of myogenic or neurogenic. In this paper we present a case of giant GIST causing remarkable displacement of the inferior vena cava.<br> A 78-year-old man visited another hospital because of a right abdominal tumor in May 2003 was found having an 18×14×11cm tumor occupied the entire right abdomen by abdominal CT scan. The tumor strongly oppressed the inferior vena cava, so that the inferior vena cava could not be visualized on CT. Echo-guided biopsy offered a diagnosis of ‘so-called GIST’, and the patient was referred to the hospital. The tumor was removed with combined wedge resection of the duodenum associated with right nephrectomy and reconstruction was made by Billroth II method after gastrectomy. The inferior vena cava was able to be separated from the tumor and was kept intact after the removal of the tumor.
