We report a case of an inguinal bladder hernia. A 78-year-old man was admitted because of voiding pain and a right inguinal soft tumor, which was diagnosed as an inguinal hernia. Abdominal US revealed prostatic hypertrophy and gall bladder stones. The laparoscopic cholecystectomy and hernioplasty were carried out. At the surgery, we found a right femorocele and the extraperitoneal tumor, 12×10cm in size, in the right inguinal canal. It was suspected to be a bladder. Cystography revealed a large inguinal bladder hernia. It was impossible to replace the herniated bladder in the normal position, so it was resected, and hernial hiatus was successfully treated by the mesh plug method. A preoperative diagnosis is very important to prevent intraoperative injury to the bladder. At treatment of an inguinal hernia or a femoral hernia, bladder hernia should be considered. It is important to take a history of a patient's urinary symptoms and an examination of US.
