Abstract

A 65-year-old man presented to a local hospital because of diarrhea and abdominal pain. Following colonoscopy and abdominal CT, rectal cancer, ileus and a retoroperitoneal abscess were diagnosed. He was referred to our hospital. We performed percutaneous dranage for the retroperitonial abscess, followed by only transient colostomy because the rectal cancer had invaded the bladder and the left wall of pelvis, and his general condition was poor due to the abscess and ileus. A tolal of 40 Grey IMRT (intensity-modulated radiation therapy) with UFT + UZEL chemotherapy for 28 days was given. After the therapy, CT, FDG-PET and colonoscopy revealed remarkable shrinkage of the tumor. High anterior resection of the rectum with D3 lymphadenectomy and partial resection of the bladder were then performed. Histopathological examination showed complete regression of the tumor, rated as Grade 3, pathologic complete response (pCR).
