A Case of Spontaneous Mesenteric Hematoma Diagnosed Preoperatively as an Ovarian Tumor

Abstract

A 53-year-old woman was referred to our hospital for detailed examination of an ovarian tumor accompanied by abdominal pain, which had been detected incidentally 1 year ago. CT and ultrasonography (US) revealed a cystic tumor in the pelvic space. The tumor was more than 10 cm in diameter. The patient underwent laparotomy on suspicion of a right ovarian tumor. However, a cystic mass was found in the small intestinal mesenterium. It was removed, along with the appendix and part of the ileum, which were adhered to the mass. The mass was identified hisopathologically as an old hematoma. The patient had no recent history of abdominal trauma, anticoagulant therapy, or vascular disease. Therefore, she was diagnosed with spontaneous mesenteric hematoma. Using CT or US, it is difficult to distinguish an old hematoma from an ovarian tumor in the pelvic space.<BR>Although spontaneous mesenteric hematoma is a rare type of lesion, it should be considered in the differential diagnosis of cystic neoplasm of the pelvic space.
