We report our experience with a case in which an incarcerated obturator hernia caused perforation of the small intestine, resulting in femoral abscess. The patient was a 94-year-old woman. While she was hospitalized for pneumonia, ileus symptoms developed. She was thus referred to the department of surgery. Because abdominal CT revealed right obturator hernia and abscess spreading to the right femur, emergency surgery was performed. According to intraoperative findings, the small intestine was incarcerated in the right obturator foramen as a Richter's hernia and showed perforation. An abscess formed and extended from the perforated lesion to the deep region of the femur. We performed partial resection of the small intestine, hernia orifice repair, and drainage of the right femoral abscess. The postoperative course was uneventful, and the patient was discharged on the 71st hospital day. We report our experience with a rare case of obturator hernia that caused a deep femoral abscess.
